Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.Program Description:The Young Adult Assertive Community Treatment Team is a unique and nontraditional licensed outpatient psychiatric clinic for individuals, 18-25 years old with Serious Mental Illness (SMI) who have demonstrated a high service need with multiple hospitalizations, emergency room visits and difficulty maintaining stability in the community. ACT is a nationally recognized model and an evidence-based practice. The ACT Team serves all of Manhattan and occasionally the outer boroughs.Purpose of Position:The Team Leader provides administrative and clinical support for the ACT team.Salary Range:$68,640.00 - $75,000.00Roles, Responsibilities, and Essential DutiesProvide case management servicesCarry a small caseload.Perform client assessments and formulate treatment goalsFacilitate client participation in entitlement programs, psychiatric services, medication management, rehabilitation and other activitiesRefer clients to community health, substance use psychiatric and other servicesAccompany clients to appointments as neededCommunicate regularly with clients' families about their needs and progressServe as liaison to other service providersMaintain up-to-date client case records according to program guidelinesVisit clients in residential facilities, on the street and in other locations and advocate with & on the client's behalf.Rotate on-call service outside of work hoursManagement, SupervisionProvide clinical supervision to staff and interns; evaluate their work and discuss progress of their clients, teach invention and advocacy skillsAssist in recruitment of ACT staff membersAssist with program administration, including coordinating staff schedules and overseeing adherence of staff training requirementsAssist in ensuring staff is adequately trained according to OMH and ACT standardsPerform Program Director's duties in his/her absenceParticipate as an ACT team memberAttend staff meetings, case conferences, and training programs to upgrade skills and knowledge.Attend supervision meetings to review client progress and case management strategiesWork collaboratively with ACT team staff in a team case management modelPolicy and ComplianceAssist in the development and adherence of Policies and Procedures in accordance with Goddard Riverside, New York State Office of Mental Health and Department of Health and Mental HygieneOther duties as requiredQualifications/Educational RequirementsLicensed Master Social worker (LMSW) requiredSIFI (supervisor in field instruction) strongly preferredSkills, Knowledge and AbilitiesExperience working with the seriously and persistently mentally ill population requiredExperience working with Young Adult population preferredACT Team experience preferredAbility to speak Spanish, French, Creole, American Sign Language or other languages is a plusComputer Skills: To Perform this job successfully, an individual should beAble to work in the Microsoft Office Suite, should especially be proficient in Word and OutlookAble to use or learn to use AWARDS databaseAble to use or learn other electronic databases, programs, and funder-related software as neededPhysical RequirementsWalking, climbing stairsWork EnvironmentField & Office workEmployee Benefits :Medical/Dental/VisionLife Insurance/Commuter benefits/Employee Assistance Program (EAP)/Flexible Spending Accounts403B Thrift Retirement Plan12 Annual Sick Days12 Agency Holidays20 Days or 4 Weeks of Paid Time off and an additional week (5) days after 5 years of employmentTotal of more than 6 weeks (a month and half) of paid time offWe are committed to building an inclusive workplace community grounded in respect for differences.
