Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.Breaking Ground's Street to Home program makes regular contact with more than 2,000 street homeless individuals and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other essential supports each year. The caring, individually tailored attention clients receive at each stage of their journey from street to home ensures that more than 95% of people who Breaking Ground places remain stably housed.Reporting to the Assistant Program Director, Outreach , t he Team Leader , Outreach , works with street homeless individuals livin g in Queens. T he Team Leader supervise s a team of Housing Advocates who utilize Harm Reduction and Housin g First approaches when conduct ing street outreach and engage with individuals to determine homelessness history, services needed, and appropriate placement options .The Team Leader is responsible for gaining a holistic understanding of the homeless population in their catchment areas and work s in collaboration with the community and other partners to implement appropriate housing and service solutions. The Team Leader manages their team respon se to community concerns , oversees Code Red/Blue alerts , and identif ies new hotspots and encampments .This position has a 35-hour work week.ESSENTIAL DUTIES AND RESPONSIBILITIES :Supervise Housing Advocates including creating schedul es , having weekly supervision meetings, providing guidance and clinical direction, and ensuring timely and effective street outreachServe as a liaison with community partners and in close collaboration, follow up on homeless conditions , community concerns, including 311 calls and encampment engagementDevelop strong collaborative relationships with external partners and service providers, NYPD, EMS, and other stakeholdersOversee and participate in Code Blue/Red AlertsEnsure documentation and reporting is accurately reported on timeAttend onsite meetings with community partnersPerforms other duties as assignedMINIMUM QUALIFICATIONS:High School diploma or equivalent required; Master's degree in related field preferred (e.g., Social Work, Public Health)Minimum of 3 years of experience with related populations; previous supervisory and administrative experience preferredUnderstanding of Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systemsExperience with crisis intervention, including, risk assessments and incident managementAbility to communicate (verbally and written) and work with diverse populationSuperb collaborative skills; Ability to work successfully with a wide range of internal and external stakeholder sSuperb collaborative skills; Ability to work successfully with a wide range of internal and external stakeholdersDetailed oriented and can manage team tasksValid NY or NJ driver's license with a good driving record, as defined by Breaking Ground, is a condition of employmentProficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft TeamsBilingual preferredEOE/M/F/Vet/DisabledOther detailsPay Type HourlyMin Hiring Rate $30.21Max Hiring Rate $30.21Apply NowS2H BQ - QUEENS, 100-30 Atlantic Ave, Jamaica, New York, United States of America
