Details Client NameSullivan County Health Care Job TypeTravel OfferingNursing ProfessionLPN / LVN SpecialtyLPN/LVN General Job ID30042733 Job TitleTemp - Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care (LTC) (Evenings) Claremont, NH Weekly Pay$1760.0 Shift Details ShiftMid - 8x5 - 03PM Scheduled Hours40 Job Order Details Start Date01/21/2025 End Date04/22/2025 Duration13 Week(s) Job DescriptionJob Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)Job Specialty: Long Term CareJob Duration: 13 weeksShift: Evening Shift, 8 hours, 40-hour guaranteeGuaranteed Hours: 40Experience: 3+ years current experienceLicense: Current NH licenseCertifications: BLS CertificationMust-Have:- Candidates must have their own form of transportation- Covid Vaccination Series and Booster RequiredJob Description:- Observe each resident on a continuous basis, alert to changes in mental and physical well-being, and notify the Supervisor of pertinent observations.- Accurately receive, transcribe, and carry out physician's orders.- Administer medications and treatments as ordered by the physician and document via computerized systems.- Supervise LNAs in accordance with licensing requirements and assist direct-care staff with nursing care.- Accompany physician on resident rounds to address medical needs and facilitate communication.- Administer oxygen and gastrostomy/jejunostomy feedings per facility guidelines.- Perform wound care and handle wound vacs.- Complete routine unit duties, such as checking appointments, ambulation lists, and documenting nutritional charts.- Ensure oversight of infection control practices on the unit.- Admit, discharge, and transfer residents, managing required documentation.- Maintain consistent communication with the Nurse Manager or Nursing Supervisor, differentiating between urgent and non-emergency issues.- Communicate with residents' families to resolve concerns and obtain necessary consents.- Manage assigned unit during shift under the direction of the Nursing Supervisor or Director of Nursing Services.- Follow regulations for controlled medications, conducting counts and reporting discrepancies.- Assist with the orientation of new nursing employees and support staff education and training.- Assign and evaluate nursing staff, initiating disciplinary actions if necessary.- Willingness to float to different units.- Must be prepared to work every other weekend and provide coverage during holidays if needed.Note: Requested time off during this assignment must be noted and verified prior to start.
