Principal Responsibilities:Position Overview:
- The YM &YWHA of Washington Heights and Inwood is seeking out a passionate and self-motivated individual to work as a temporary social worker for the Aging In Place Outreach Network (APON), the Y's new non-traditional referral network into its highly respected continuum of care for older adults in Upper Manhattan.
- The Outreach Coordinator will require both strong clinical assessments skills as well as excellent community outreach abilities.
- This is a temporary position while a full-time employee in on leave.
RESPONSIBILITIES include, but are not limited to the following:
- Conducting outreach and establishing connections within the community.
- Developing training materials for local networks of community members with no background in geriatric care on identifying signs that an older adult may need support age in place safely.
- Referring older adults in need of support to the appropriate Y aging in place services.
- Conducting home visits for initial assessments and care planning.
- Providing supportive counseling.
- Maintaining electronic records.
- Tracking data and trends through an online relational database.
- Building relationships with community partners.
POSITION TYPE / EXPECTED HOURS OF WORK:
- Temporary position.
- 40 hours per week.
WORK ENVIRONMENT:
- Work is performed in a typical community center environment.
- Home visits with program clients is required.
- Outreach to community sites including but not limited to NORC buildings, houses of worship, police stations, fire houses, and local government offices.
COMPENSATION:
- Hours worked paid at $30 per hour.

Minimum Qualifications:- LMSW required, LCSW preferred.
- Bi-lingual in English and Spanish preferred.
- Minimum of two years of experience working with the geriatric population - a background in community outreach preferred.
- Documentation of U.S. citizenship or eligibility to work in the U.S.
- Must pass the background check and other pre-employment requirements.
Salary:Under $25,000
