Position: Temporary In-home services Program CoordinatorLocation: Long Prairie, MNJob Id: 7416# of Openings: 1Meridian ServicesWhere Opportunities and Dreams Align!Join our team as a TEMP Program Coordinator in our Individualized Home Supports North ProgramWage: $22 per hour plus a full benefits package to include paid volunteer timeDuration: Temporary position ending in January 2025Schedule: Monday - Friday typically day hours with availability for evenings and weekends based on needs of the program and staffing. Our mission is to provide services of the finest quality, so that those we serve will be able to realize their dreams.JOB SUMMARY:Opportunity to manage a caseload of In-Home Service individuals in Long Prairie & surrounding areas. The people we serve on this caseload have intellectual disabilities and/or mental health diagnosis. This caseload oversees approximately 15-20 persons served. Our persons served within In Home Services receive 1:1 supports from direct care staff in their homes and in the community. Responsibilities include supervision and training of in-home staff, oversight of persons served care programs, conducting meetings, licensing regulations, and direct care. This position offices out of the Long Prairie Group Home and does consist of traveling approximately 50-60% of the time based on program needs of training staff, working direct care, attending meetings etc.ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:Supervision of Direct Support Professionals (DSP's)Train Direct Support Professionals, run monthly staff meetings, write and complete evaluations for DSP'sLead meetings with parents/guardians, teachers and/or work programs, and case managersAssist with the development and implementation of person-centered plans and outcomes including writing progress reviews and making recommendations to the team on changesAssist in writing behavioral programming with Administrator as applicable per person servedCompletion of person served paperwork in compliance with 245DUtilization reports on budgets and actuals for each person's budgeted hoursCompletion of monthly schedulesPayroll and Expense sheets for staff- biweeklyDevelop and connect with community resources to ensure successful community integrationSKILLS/ABILITIES:Writing skills adequate to write narrative accounts of daily events and emails to team members.Excellent communication with all team members via email, text, phone calls and in person as dictated by each person's team. Must be able to communicate with a variety of persons of different backgrounds and educational and life experiences.Great time management, problem-solving skills, and work ethic.Ability to multi-task and meet deadlinesExperience working as a DSP or PCAMust present a professional appearance and promote a positive work environment.PREFERRED EDUCATION AND EXPERIENCE:Experience supervising staff in a social service fieldCertificate or bachelor's degree in human services or related area.Requirements:Must be at least 18 years of ageProficiency in spoken and written English communicationHave a vehicle with current car insuranceHave an acceptable driving record as determined by Meridian Services policy.Successful clearance from a DHS Background StudyExperience working direct care with individuals with intellectual disabilities and/or mental health diagnosis.Prefer experience with supervisionComfortable with personal caresQualified candidates are encouraged to apply at /careers/.
