Job Description: - Demonstrate the knowledge and skills necessary to provide care appropriate to adult and geriatric patients.- Assess and interpret data to understand each patient's age-specific needs.- Provide direct and indirect skilled nursing care for assigned residents.- Follow defined policies and procedures under the supervision of a Registered Nurse.- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff.- Participate in continuing education and in-service programs to maintain clinical expertise.- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.- Contribute to creating age-appropriate and culturally relevant plans of resident care.- Recommend revisions to care plans after evaluating resident outcomes and indicate the need for referrals.- Collaborate with residents, families, and the Interdisciplinary Team to ensure customer satisfaction.- Participate in mentoring and orientation of staff and consider factors related to safety, cost, and impact in care delivery.- Provide leadership in professional practice under the direction of the RN.- Maintain knowledge and competency in nursing practice and handle occupational exposure to bloodborne pathogens responsibly.
