Summary of the Position: The Individual Service Option (ISO) In Home Therapist provides therapeutic and clinical services to children and caregivers as needed and outlined in Individualized Service Plans as part of the Merrimack County Intensive Service Option (ISO) In-Home program.
* Services include individual, family, group, and substance use disorder counseling as needed.
* All therapeutic services and service plans are based on strength-based approach using the wrap around model.
* Therapist maintains a caseload of no more than 6 families per month on average and is required to provide a minimum number of face-to-face hours with families each week.
* Services are provided in the homes and communities of the families served at the times and locations convenient to them.
* Therapist is supported by and provides supervision to Advocates/Case Managers conducting case management activities.
* Documentation must be completed in accordance with New Hampshire Medicaid standards.
* Applicant must be dependable, committed, and able to serve as a positive role model for youth in community, school, and home settings.
* Position offers flexible hours, competitive weekly pay, and activity reimbursement.
Qualifications/Requirements: A master's degree with a major in social work, counseling, psychology, or a related field and at least 2 years of direct work experience in assisting children and families; or a bachelor's degree with a major in social work, counseling, psychology, or a related field and at least 5 years of direct work experience in assisting children and families.
* Demonstrated communication skills, both oral and written. Experience in the provision of home and community-based services.
Position requires reliable transportation, valid driver's license, and current auto insurance coverage.
