DetailsClient Name Beacon Hospice - Bangor Job Type Travel Offering Allied Profession Therapy Specialty PT Job ID 28574077 Job Title Therapy/Rehabilitation - PT Home Health @ Amedisys Beacon - Bangor Weekly Pay $2286.0 Shift DetailsShift Day - 8x5 - 09AM Scheduled Hours 40 Job Order DetailsStart Date 11/25/2024 End Date 02/24/2025 Duration 13 Week(s) Job Description Job Title: Physical Therapist - Home Health Job Specialty: Home Health Job Duration: 13 weeks Shift: Day, 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum of 1 full year Adult Home Health Experience or 1 full year Hospice experience required License: Must have a valid license in hand Certifications: CPR, BLS required Must-Have: - Proficiency in HomeCare HomeBase EMR, used within the last 6 months - Proficiency in OASIS for RNs, PTs, and OTs - Reliable transportation with a valid driver's license and agency-required liability insurance - Willingness to travel up to 75 miles from the home clinic Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physician orders and agency policies and procedures through the competent application of the nursing process. - Responsible to the Clinical Manager, directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Expected to handle an average workload of 5-10 patients per day, equating to roughly 6-8 points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Territory coverage includes areas from Bangor Northwest to Dover, and Bangor Northeast to Lincoln. - Dress Code: Scrubs required. - Maintain proficiency in EMR systems used for documentation and charting. - Open to a weekend rotation approximately once a month; no on-call requirement. - Must be open for on-call rotations with prior discussion during the interview. - Local candidates are accepted at the same rate.
