Job Title: Physical Therapist - Home Health Job Specialty: PT Home Health Job Duration: 13 weeks Shift: Day, 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency with HomeCare HomeBase (HCHB) and OASIS - Reliable transportation with a current driver's license and agency-required liability insurance Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and periodic office meetings; must be willing to travel up to 75 miles from the home clinic.- Average 5-8 patients per day, aiming for approximately 30 points per week; each type of visit and activity holds a different point value. - Handle types of cases including Start of Care, Wound Care, and Follow-up visits.- Be open to on-call rotations, approximately one weekend per month.- Work in both care facilities and patients' homes.- Discuss weekend requirements and on-call rotations during the interview.- Attend the primary location and may cover other territories as necessary.- Orientation to be completed during compliance if HCHB proficiency isn't recent.- Local candidates accepted at the same compensation rate; pending licenses are not accepted.
