Job Title: Physical Therapist - Home Health Job Specialty: Therapy/Rehabilitation Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not Guaranteed Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience License: Must have a license in hand Certifications: CPR, BLS Must-Have: - Proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months- Experience with Oasis for RNs, PTs, and OTs Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Travel for home visits and periodic office meetings; must have reliable transportation, a current driver's license, and agency-required liability insurance.- Average number of patients per day: 5-8.- Assigned between 5-10 patients per day, equating to roughly 6-8 points.- Provide different types of care, including Start of Care, Wound Care, and Follow-up Visits.- Must be willing to travel up to 75 miles from the home clinic; mileage reimbursed at the IRS Standard rate.- No weekend or on-call requirements in most locations; discuss specifics during the interview.- Experience working with PointCare EMR systems.- Cover territory surrounding the specified care center to assist with workload due to a lack of full-time PT.- Locals accepted at the same rate.Please ensure your application meets all necessary requirements and experience levels, as outlined above, to be considered for this role.
