Job Title: Physical Therapist Job Specialty: PT Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year of Adult Home Health or Hospice experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: - Proficiency with HomeCare HomeBase EMR system used within the last year- Experience in home health- Willingness to travel up to 75 miles from the assigned care center- Must not be a current direct employee of any related location if applying as a traveler Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.- Responsible to the Clinical Manager and directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and attend periodic office meetings; reliable transportation with a current driver's license and agency-required liability insurance is essential.- Handle various cases including Start of Care, Wound Care, and Follow-up Visits.- Manage an average of 5-8 patients per day, translating to approximately 6-8 points based on diverse activities such as documentation and travel.- Coverage includes Baltimore County and Baltimore City with travel ranging between 50-200 miles per week.- Efficient charting using HomeCare HomeBase (HCHB) and demonstrated use of Oasis is required for RNs, PTs, and OTs.- No weekend rotation or on-call duty unless otherwise discussed during the interview.Interested candidates should apply with the prerequisite experience and credentials as detailed above.
