Job Title: Physical Therapist (PT) - Home Health Job Specialty: PT Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Per contract, facility may adjust shifts as needed Experience: 1 year of Skilled Home Health Experience required License: Pending license accepted Certifications: BCLS Must-Have: Primary series COVID vaccination and one booster Job Description: - Provide skilled physical therapy services in a home health setting.- Expected to conduct up to 6 patient visits per day with a standard productivity requirement.- Maintain a 1:1 patient ratio during visits.- Use EPIC Remote Client for Home Health and Rover for charting.- Ensure proper grouping of clients to minimize travel time, with a maximum daily radius of roughly 60 miles.- Weekend work is not required; no on-call duty.- Own transportation is necessary for travel between client locations.- Flexible with floating between facilities within the state, as needed.- Opportunity is open to first-timers; local candidates accepted within a specified radius.- Completion of required orientation and training modules prior to start.- Dress code is business casual; scrubs are optional and no specific color is required.
