Weekly Pay Range - $2332 To $2591 Job Title: Physical Therapist - Home Health Job Specialty: PT Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Confirmed as per contract Experience: 1 year of skilled home health experience required License: Pending license accepted Certifications: BCLS Must-Have: Own transportation required Job Description: - Provide home health physical therapy services in Sonoma, CA and surrounding areas.- Work with a caseload of up to 6 patients per day, maintaining a 1:1 patient ratio.- Complete 30 productivity points as a standard for patient visits.- Orientation is 40 hours before commencing patient care.- Require availability for one weekend shift per contract.- Use EPIC for charting; knowledge of Oasis is preferred but not mandatory.- Mileage reimbursement per IRS standard for work-related travel.- Candidates should be open to working within a 50-mile radius.- Floating between all facilities within the state may be required.- Module completion averages 16 non-billable hours and is required pre-start and annually.- Provide own communication device or submit monthly cell phone bills for reimbursement up to a specified amount.- Medical and religious exemptions for COVID-19 vaccine are allowed.
