The organization is dedicated to impact the community and focused on providing resources and opportunities to improve outcomes. East Side House has identified that education is the foundation for economic improvement and through its youth education and technology programs it teaches skills to allow its students and clients to become more self-sufficient and independent. As a community resource the organization also provides services to families and a growing older adults population. During the COVID 19 pandemic the organization began food distribution as an additional way to provide economic support to the community we serve. ESHs Older Adults Program meets the needs of older adults by providing a variety of social, protective, and supportive services in a non-medical care setting. The program improves the quality of life for seniors in the community by offering opportunities for personal achievement, social engagement, and community participation. Additionally, the program includes nutrition education, a congregate meal program, counseling, educational activities, transportation assistance and benefits assistance. Recreational activities that address the cultural diversity, health and safety of seniors are also incorporated into the program, such as bingo, dominoes, monthly trips, special events, arts and crafts, health screenings, walking club, computer club, Facebook club and birthday parties.As a member of the Older Adults team, and under the supervision of the Program Director, the Title B Case Worker will provide services to participants at East Side Houses Borinquen Court Mitchel Senior Center, located at 295 East 138th Street, Bronx NY 10454.Primary FunctionsDuties and Responsibilities shall include but are not limited to the following:
- Conduct intake and assessment of the older adults to be enrolled in the DFTA program.
- Conducts and collaborates in process of assessment, planning, evaluation, and advocacy for options and services to meet the program participants needs through communication and available resource to promote quality, cost-effective outcomes.
- Maintain up-to-date participants database and file. Assist in the coordination of recreational and other activities older adults enroll in.
- Provide case management services in the areas of Advocacy, Information & Referral, Benefit & Entitlement, Immigration assistance, Supportive Contact, PHQ-9 screenings, and counseling.
- Make use of Peer Place to input data of related services received
- Communicate via telephone and/or letter with participants for follow up-and reminder.
- Attend in service trainings as assigned by ESH and DFTA.
- Update the community resource guide used for referrals
- Assist with program assessment, preparation, and follow-ups
- Submit monthly case assistance and rosters report.
- Cover Activities Coordinator as needed.
- Support as Task Supervisor of the Social Work Interns

Expected Work ScheduleThis position is a full-time position expected to work thirty-five (35) hours a week. Generally, the work schedule for this position is 8:00am-4:00pm, Monday - Friday. Flexible work schedule is required for Summer Cooling center days/hours. This job operates in a professional office environment located in a public community space. - Minimum High School diploma with two years of case assistance experience
- Associate degree or above in Human Services or related area strongly preferred
- Case management experience preferred
- Bilingual Spanish
- Excellent skills in verbal and written communication
- Demonstrated knowledge of community service/development
