About IHTC At the Indiana Hemophilia & Thrombosis Center (IHTC), we offer a future where people with rare blood disorders flourish. What You Will Do As a Transition Advance Practice Provider (APP), you will medically manage patients with chronic hematological issues between puberty and young adulthood in collaboration with the IHTC physicians.
The Opportunity - Triage/management of patient issues in the following areas: on-call, inpatient, and other outpatient setting.
- Patient management includes but not limited to health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, education/counseling, ongoing monitoring and follow-up.
- Documentation in electronic medical record per policies of IHTC and St. Vincent Hospital.
- Assist with planning and implementation of community programs related to transition such as summer camp and patient/family events.
- Work with patients to successfully transition from the pediatric medical providers to adult providers at the appropriate age/developmental level.
- Perform procedures, as delegated with competency check off by the Physicians.
- Authorized to administer Basic Life Support in accordance with current CPR standards.
Requirements - Minimum Master's degree
- Minimum 3 years of experience working with patients with a chronic disease
- Valid IN Nurse Practitioner or Physician Assistant (and, if applicable, PA-C) License
- Valid driver's license and insured automobile required
