About IHTC At the Indiana Hemophilia & Thrombosis Center (IHTC), we offer a future where people with rare blood disorders flourish. Our experts provide the highest quality comprehensive services and holistic care to patients with bleeding, clotting and other hematologic disorders, and to their families. What You Will Do As a Transition Advance Practice Provider (APP), you will medically manage patients with chronic hematological issues between puberty and young adulthood in collaboration with the IHTC physicians. The Opportunity
  • Triage/management of patient issues in the following areas: on-call, inpatient, and other outpatient setting.
  • Patient management includes but not limited to health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, education/counseling, ongoing monitoring and follow-up.
  • Documentation in electronic medical record per policies of IHTC and St. Vincent Hospital.
  • Assist with planning and implementation of community programs related to transition such as summer camp and patient/family events.
  • Work with patients to successfully transition from the pediatric medical providers to adult providers at the appropriate age/developmental level.
  • Perform procedures, as delegated with competency check off by the Physicians.
  • Authorized to administer Basic Life Support in accordance with current CPR standards.
Requirements
  • Minimum Master's degree
  • Minimum 3 years of experience working with patients with a chronic disease
  • Valid IN Nurse Practitioner or Physician Assistant (and, if applicable, PA-C) License
  • Valid driver's license and insured automobile required
