BS - Bachelor of Science nursing preferred. 1 year of experience in long term / sub-acute care setting or LTC admissions required. (This requirement only applies to LTC setting) 1 year of experience in medical acute rehab within the past 5 years for medical acute rehab settings only preferred. (This requirement only applies to Acute Care setting preferred) Registered Nurse required upon hire. FIM Function Indp Meas required within 90 days. FIM Certification (Functional Independence Measure) - obtained through successfully passing the Licensed Personnel Exam through the on line credentialing system on Uniform Data System for Medical Rehab (UDSMR.org), within 90 days of hire or appointment to the position, for medical acute Rehab settings only. REQUIRED - Certification must be renewed every 2 Years. Patient Review Instrument (PRI)required within 90 Days.
