Penn State Health - Hershey Medical CenterLocation: US:PA:HersheyWork Type: Full TimeFTE: 1.00Shift: DayHours: 8:00a - 4:30pRecruiter Contact: Hector Diaz at [email protected]SUMMARY OF POSITION:Under the direction of an RN Care Coordinator (RNCC) or Masters prepared Social Worker (MSW) the purpose of this position is to assist in communicating the discharge plan to patients and families and facilitating safe and timely transitions of care and services from acute care to alternative levels such as skilled nursing facility, inpatient rehabilitation, and/ or home program. This role will work collaboratively with the care transitions team members, providing onsite face to face interactions with patients and families and telephonic discharge arrangements with external entities and post-acute vendors.
