Job Summary:
The Transitional Care Coordinator provides support to Nursing Case Managers and Social Workers. Conducts insurance benefit checks, arranges durable medical equipment, manages eDischarge, arranges transportation, assists with prescriptions and accessing emergency funding. Works closely with colleagues and provides back up coverage. Responsible for maintaining Transitional Care Office. Assist with onboarding new staff and post acute partners.
Minimum Qualifications:
Associates degree in Business or Health Related Field and 3 years relevant experience (preferably in a health related setting). Excellent written/oral communication, organizational and interpersonal skills required.
Preferred Qualifications:
Working knowledge of computers and book/record keeping skills desirable
Work Days:
Saturday and Sunday 8:00 am-4:30pm
