We are seeking a Transitional Care Nurse to join our team!

Responsibilities:

Coordinates the clinical, technical and operational services for a dedicated patient population. This is accomplished through expert nursing practice in the areas of care giving, coordination, consultation, education, and research. Expedites patient education, support/satisfaction, and placement, scheduling and follow-up to provide the most effective, efficient delivery of quality care across the continuum of healthcare services. Works in collaboration with the multidisciplinary team (physicians, support staff, administration, marketing and outreach) and develops, maintains and evaluates the success of the program under the direction of the Director of Nursing. Maintains quality assurance data and develops reports for distribution to appropriate parties. Serves as a program liaison to physicians, patients, consumers, payers, co-workers, community and outside organizations. Demonstrates commitment to and support of the Mission, Vision, and Values of VibraLife.

Required Skills:

  • Current, valid, and active license to practice as a Licensed Nurse in the state of employment required.
  • Current BLS certification from a Vibra-approved vendor required.
  • Minimum three (3) years of experience in medical-surgical and/or SNF nursing required.
  • Minimum one (1) year of supervisory experience required

Additional Qualifications/Skills:

  • Registered Nurse preferred.
  • Three (3) years experience in an administrative position within a Skilled Nursing Facility preferred.
  • Current ACLS certification from a Vibra-approved vendor preferred.
  • Experience and knowledge of operations, including staffing and scheduling, budget management, workflow planning and staff development.
  • Ability to manage the practice of nursing within the mission of the facility.
  • Demonstrated ability to participate in recruitment and hire of qualified staff.
  • Ability to project a professional image.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing and analytical skills.
  • Ability to make independent decisions when circumstances warrant.
  • Working knowledge of computer and software applications used in job functions.
  • Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
