Job Details - Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit.
- The duties require the use of tact and judgment in the care and treatment of patients.
- May assist in the supervision of non-professional subordinates.
- Is cognizant of philosophy, standards, objectives and policies of the Nursing Department.
- Performs related duties as required.
Job Requirements - Demonstrates proficient knowledge in the treatment of patients requiring nursing care.
- Uses available diagnostic results and other information to make an accurate evaluation.
- Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process.
- Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted.
- Is considerate of residents' privacy during the evaluation process.
- Contributes to an individualized and appropriate plan of care delineating measurable patient care goals.
- Contributes to the plan of care making necessary changes as the condition of the patient indicates.
- Bases care decisions on resident needs and care priorities.
- Monitors patient outcomes.
- Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement.
- Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner.
Additional Details - Minimum one (1) year clinical experience preferred.
- Ability to work in surroundings with a degree of dust, noise, etc. without undue discomfort.
- Ability to hear, talk, walk, stand, bend and stretch.
- Ability to read, write and interpret documents.
- Ability to sit for long periods of time at desk to perform job functions.
- Ability to stand for long periods of time at required patient bedside and/or procedural areas.
- Ability to communicate effectively with patients and/or co-workers.
- May be exposed to mechanical/electrical/radiant/explosive/burn and chemical hazards.
- May be required to handle, dispose of and/or identify hazardous waste and hazardous materials.
- May be required to operate and clean equipment.
- May be required to handle/assist in lifting patients onto/out of exam beds/procedure stations.
- Must be trained in and knowledgeable of all policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work.
- Knowledge of all codes and demonstrate understanding.
- Duties performed routinely require exposure to blood, body fluid and tissue.
- Normal routine involves no exposure to blood, body fluid or tissue, but potential for exposure.
