BronxCare is seeking a dedicated and bilingual (English/Spanish) Registered Nurse (RN) to join our Outpatient Clinic team, specializing in Internal Medicine. The ideal candidate will have at least 1 year of clinic experience, a strong background in patient education and care planning, and a commitment to high-quality patient care.

Job Duties:
- Provide patient education and care planning, focusing on chronic disease management, particularly Diabetes and related conditions.
- Collaborate with the Endocrinology team and other specialists to ensure comprehensive care.
- Assist in managing patient flow and provide support to multiple providers as needed.
- Engage in care planning and ensure there are no gaps in patient care or documentation.
- Be flexible and adaptable to the changing needs of patients and the department.
- Demonstrate excellent customer service skills and work collaboratively with physicians, staff, and patients.
