Certification Details- BLS
- RN Case Manager
Job Requirements- Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience.
- Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge
- EPIC CHARTING experience is REQUIRED
- Department: CM - General Case Manager/Discharge planning
- Will cover all Inpatient units throughout the hospital
Schedule Information- 5x8hr shifts - non-negotiable
- Shifts & Scheduling ? M-F 8-430p
- Weekend Requirements None
- Holiday Requirements Rotated as needed
Unit Specific Information- Uniform: Any color scrubs or business professional attire
- RT for DC Planning? Will consult with RT as need
- Pharmacy for DC Planning? Will consult with Pharmacy as needed
- Case load/# patients per day/consults per day 15-20
- Common types of patients & job duties Medical discharge (could be some social discharges)
Additional Details- 50 mile radius rule for travelers
- FLU VACCINE IS REQUIRED at UMMC - only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccine
- FINGERPRINT REQUIREMENT - Remote process must allow 10 days, In Person can be done at Jackson or Grenada campus the week prior to start
- UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.
