Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: ASAP Duration: 14 weeks 36 hours per week Shift: 12 hours, nights Employment Type: Travel Position : RN Care Manager Shifts : 3-12 hour shifts, rotating Days/Nights/Weekends, Holidays required. Coverage for Clinical Coordinator as needed. Must be willing to travel to multiple locations as requested. Requirements : NYS RN License required in hand 5 years of healthcare experience required Care/Case Management experience in a hospital setting Ability to handle confidential information with discretion Proficient in Microsoft Office (Word, Excel) Strong communication, professionalism, and teamwork skills Ability to assess and meet patients' age-specific needs (pediatric to geriatric) Responsibilities : Assess patients' needs and develop appropriate care and discharge plans Coordinate care with patients, families, physicians, and team members Ensure care is provided in the right place, at the right time, by the right provider Conduct readmission assessments within 30 days of discharge Collaborate with multidisciplinary teams to implement clinical guidelines Serve as a patient advocate and provide ongoing education Document care processes, manage risks, and address quality concerns Mentor new staff and act as a liaison to post-hospital care providers Participate in care rounds to optimize resource utilization and support family education MedicalPeople Staffing, LLC Job ID 29029342.
