The staff member mustdemonstrate the knowledge and skills necessary to provide care appropriate tothe age of the patients served on his or her assigned unit. The individual mustdemonstrate knowledge of the principles of growth and development of the lifespan and possess the ability to assess data reflective of the patient's statusand interpret the appropriate information needed to identify each patient'srequirements relative to his or her age-specific needs, and to provide the careneeded by pediatric, adolescent, adult and geriatric patient group.Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve a potential contact with blood, body fluids,tissues. Assess the biophysical and psychosocial needs ofthe patient on admission and on an ongoing basis throughout the hospital stayensuring that the appropriate level of care is assigned and maintained and thatthe appropriate discharge plan is in place. Through daily rounds on the patientcare units, ensure that the right care is provided in the right place, by theright provider, at the right time, and at the most appropriate cost. Collaborate with patient, family, physician,and team members to develop a transition plan appropriate to the patient sneeds and financial capabilities. Conducts a readmission assessment on all readmissions within 30 days ofindex admission discharge date.Identifies needed services and coordinatesreferral to appropriate agency/service provider. Works as a health team member to ensure that services areavailable and in place in a timely manner. Coordinates, as needed, amultidisciplinary care planning process for patients meeting criteria and/orare referred by other team members. Facilitates implementation of clinicalpractice guidelines; assists other professional disciplines in effectivelydeveloping and integrating support modalities into guidelines to ensure that anefficient, comprehensive plan of care is established. Provides ongoingeducation to patients and families as it relates to care planning andpreparation for discharge. Serves as apatient advocate. Enhances acollaborative relationship to maximize the patient s and family s abilityto make informed decisions. Make dailyrounds with assigned physicians based on priorities of patient s needs asavailable.Documents and reports quality issues, risk management, infection control,complaints, and efficiency issues in the provision of care; interface with thePerformance Improvement Department in concurrent and retrospective monitoringof compliance with clinical guidelines. Completesthe patient and family caregiver interview tool on all readmissions within 30days of index admission discharge date and forwards tool to Director CareManagement. Provides orientation andmentoring to new staff and students. Acts as a liaison to post hospital providers andother community health resources. Provides verbal and written communicationwith physicians, nursing and ancillary staff as necessary to ensure continuityof care. Has the ability to influence decisions, gain cooperation, andfacilitate change with physicians, all levels of management, and employees andsupervisors of other hospital departments. Is knowledgeable and followsregulatory requirements of reporting to Child Protective Services, AdultProtective Services, and other agencies. Participates in interdisciplinary patient care rounds and/or conferencesto review treatment goals, optimize resource utilization, provide familyeducation and identified post-hospital needs. Documents clearly and accurately in the medical record in accordancewith departmental, organizational and regulatory agency guidelines.Reviews for medical necessity utilizingInterQual Criteria. Demonstrates ability to identify barriers that impactpatients' length of stay and corroboratively develops and implements strategiesto decrease length of stay. Monitors appropriateness of test/procedures,consultations, treatment plans and resource utilization. Anticipatesbarriers/variances to the delivery of care; intervenes as necessary. Initiates and presents denial letters , as appropriate. Issue Medicare adverse determination letterswhen deemed appropriate.
