Job Title: Pediatric Registered Nurse - Home Care Job Specialty: CHHA Peds RN Job Duration: 13 weeks Shift: 3x8.0; Friday - Sunday, 8:00 AM - 4:30 PM Guaranteed Hours: 32 hours/week Experience: One year of experience as a clinical pediatric nurse strongly preferred; nine months of experience in home care preferred License: New York State Licensed RN Certifications: Basic Life Support (BLS), any provider accepted Must-Have: - Strong interviewing, clinical assessment, and communication skills - Critical thinking skills and ability to make appropriate clinical judgments - Ability to work independently, adapt to change, and manage assignments - Current NYS driver's license and auto insurance - Certified Home Health Care Experience required Job Description: - Develop care plans based on patients' and families' healthcare needs in collaboration with physicians, clinical teams, and third-party payers. - Complete OASIS Assessments to accurately reflect clinical, functional, and social needs. - Ensure compliance with state and federal legislation by completing all appropriate clinical records within established time frames. - Refer to appropriate healthcare systems and community agencies/resources as needed. - Communicate assessment outcomes with healthcare team members to ensure appropriate follow-up. - Maintain exemplary levels of patience, courtesy, and flexibility in interactions with patients, physicians, and staff. - Resolve issues at the point of contact and maintain appropriate productivity levels. - Adhere to Professional Home Visit Performance Standards.
