Job Title: Level 2 Registered Nurse Case Manager Job Specialty: Case Manager RN in Home Health Care Job Duration: 13 weeks Shift: Monday to Friday, 8-hour shifts (Day shift: 8:00 am - 4:30 pm), weekends as needed Guaranteed Hours: Not specified Experience: One year of experience as a clinical nurse in a hospital required, and 9 months of experience in home care preferred License: RN License (State License ONLY) Certifications: BLS (any provider accepted) Must-Have: - Certified Home Health Care Experience - EPIC or Netsmart experience preferred - Experience with OASIS assessments Job Description: - Develop and revise treatment and discharge plans in collaboration with the patient, family, physician, care team, and third-party payers. - Complete and submit OASIS assessments, reassessments, and other required documentation within defined timeframes. - Process medical orders and communicate patient needs and changes in condition to the physician. - Provide a respectful environment that promotes patient privacy, confidentiality, and property. - Initiate appropriate preventive and rehabilitative strategies. - Deliver services requiring specialized nursing skills and educate patients and families on disease processes and preventive strategies. - Supervise LPNs to ensure patient health, safety, and compliance with the plan of care. - Coordinate and evaluate the quality of patient care services provided by the Home Health Agency. - Support care management philosophy to promote quality and cost-effective outcomes. - Ensure completion of all clinical records in compliance with state and federal legislation. - Maintain current knowledge of all available services and community-based resources. - Demonstrate flexibility in visiting patients based on need. - Ensure problem resolution at the point of contact and secure proper authorizations and releases of information. - Participate in patient care conferences and contribute to the attainment of operational and financial objectives. - Monitor quality indicators and participate in Quality Management reviews and improvements. - Promote a professional growth environment that values diversity. - Participate in education, training programs, and self-development as necessary. - Responsible for meeting Key Performance Indicators (KPIs) established by management.
