Job Summary:Seeking a Registered Nurse (RN) for the Care Management Department. The RN will be responsible for performing comprehensive assessments, ensuring proper discharge planning, and working with the healthcare team to prevent readmissions and improve patient outcomes. The RN will also handle Utilization Review (UR) activities as per department policies.
Responsibilities:Comprehensive Assessment: Conduct initial assessments on patient admission, utilizing tools to identify high-risk indicators and ensure appropriate discharge planning.
Multidisciplinary Care Coordination: Coordinate care within the hospital and across hospitals, ensuring effective communication and collaboration with the healthcare team.
Discharge Planning: Manage patient education, arrange aftercare, engage with commercial payers, and address high-utilizer patient root causes.
Collaborative Communication: Ensure relevant patient information is communicated to appropriate team members, and participate in multidisciplinary rounds.
Quality Improvement: Participate in performance improvement initiatives as needed.
Patient Care for Geriatrics: Ensure safe discharge and continuity of care for geriatric patients, including inpatients, emergency department, and surgical outpatients.
Other Duties: Perform other related duties as assigned.
License: Valid RN license in New York State (NYS).
