Job Overview:

We are seeking a Registered Nurse with a background in case management to join the interdisciplinary team at BronxCare Hospital. The role involves managing discharge planning and referrals for patients in the acute care setting, primarily on Med-Surg or Telemetry units.

Responsibilities:

• Collaborate with the healthcare team, including physicians and social workers, to develop and implement patient discharge plans.
• Manage a caseload of 17-25 patients, with a focus on safe transitions from hospital to home, community, or skilled nursing facilities.
• Handle discharge documentation and referrals for various needs, including homelessness, dialysis, wound care, and more.
• Work independently on assigned units, actively participating in patient rounds with the care team.
• Occasionally cover weekends or holidays based on departmental needs and management approval.

• Trauma Level: II
• Charting System: Allscripts
• Orientation: Includes 2 days of hospital orientation and 1-2 weeks with a preceptor.
• Dress Code: Business casual or neutral-colored scrubs (excluding Royal Blue or Maroon). Lab coat provided.
• Weekend & Holiday Requirements: Rare, with prior management approval.

Work Environment:

You will work closely with social workers and an interdisciplinary team in a supportive and collaborative setting, managing patient care on Med-Surg or Telemetry units. You'll be responsible for helping to ensure a smooth transition for patients upon discharge and for providing critical resources to those in need.
