Job Title: Registered Nurse - Home Health Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: Minimum 1 year Adult Home Health or Hospice experience License: Active RN License Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) EMR, OASIS experience Job Description: - Provide, coordinate, and direct home nursing care based on physicians' orders and agency policies - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Conduct home visits and periodic office meetings, must have reliable transportation - Perform various types of visits including Start of Care, Wound Care, Follow up Visits, etc. - Accept every patient on the schedule and complete all documentation within 24 hours of the appointment - Proficient in HomeCare HomeBase (HCHB) and Oasis EMR systems - Willing to travel up to 75 miles from assigned care center - Hit weekly productivity points of 30 equating to 40 hours per week - Flexible to float to nearby locations to pick up patients if needed - Comfortable with long drives between patients, may be longer than 20 minutes - Open to on-call rotations - No guaranteed hours, all overtime must be pre-approved Note: All information and requirements must be articulated clearly and concisely in the job description.
