Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00), Monday to Friday Guaranteed Hours: No guaranteed hours Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have license inhand Certifications: CPR, BLS Must-Have: - Recent specialty experience required - Proficiency in HomeCare HomeBase (HCHB) and OASIS - Reliable transportation, current driver's license, and required liability insurance - Ability to travel up to 75 miles from the home clinic - Completion of documentation within 24 hours of appointment Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies. - Directly supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses. - Conduct an average of 5-8 patient visits per day, totaling 6-8 points per day and 30 points per week. - Engage in home visits and periodic office meetings. - Handle types of cases including Start of Care, Wound Care, and Follow-up Visits. - Participate in weekend rotations and on-call duties as required. - Cover Sussex County territory with an estimated travel of 150-200 miles per week. - Ensure all patient documentation is accurately completed within 24 hours. - Attend to weekly clinic meetings and manage patient visits efficiently. - Willingness to float to nearby clinics if patient census is low to meet financial needs. This position requires applicants to be proficient in using specific electronic medical record (EMR) systems, such as HomeCare HomeBase, and to meet weekly productivity expectations while maintaining high standards of patient care and compliance with healthcare regulations.
