Job Title: Registered Nurse (RN) Job Specialty: Home Health Job Duration: 260 Days Shift: Day - 5x8 - 9am - 5pm Guaranteed Hours: Experience: Two (2) years in a HOME setting within the last fifteen (15) years License: Current RN Licensure in New York State Certifications: BLS required Must-Have: Recent experience conducting comprehensive assessments of patients Job Description: - Conduct initial in-home pre-evaluation of patients, evaluating health status, care needs, and preferences to develop individualized care plans. - Review and enter evaluation data into electronic forms, document concerns or issues encountered during assessments. - Communicate with consumers about healthcare options, services, and support needed. - Collaborate with interdisciplinary care teams to enhance care coordination and service delivery. - Maintain knowledge of community resources, payor requirements, and network services for the target population. - Identify opportunities for health promotion and illness prevention. - Document case notes and collect quality review data for outcome measurements. - Perform assigned duties as directed by management. - Ensure compliance with standards and performance criteria for the position. - Other responsibilities as assigned by the Regional Nurse Manager.
