Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No guaranteed hours Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) and OASIS, used within the last 6 months, or willingness to complete a retraining course - Reliable transportation, valid driver's license, and agency-required liability insurance Job Description: - Provide, coordinate, and direct home nursing care based on physician's orders and agency policies - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Conduct home visits and attend periodic office meetings - Responsible for driving to patients' homes daily, covering a mileage radius of up to 75 miles from the home clinic - Manage cases such as Start of Care, Wound Care, Follow-up Visits, etc. - Expected to see approximately 5-10 patients per day, equating to roughly 6-8 points - Maintain documentation completed within 24 hours of patient appointments - Meet daily and weekly productivity expectations (30 points per week) - Travel compensation follows the IRS Standard rate for patient home to patient home - Open to being floated to nearby clinics if patient census is low - Dress Code: Scrubs Additional Information: - No on-call and weekend duties are expected unless absolutely necessary - The workweek schedule is Monday through Friday with standard working hours Please ensure your readiness for travel, documentation responsibilities, and patient interactions as described.
