Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Per contract, facility can call off up to one shift per pay period if needed Experience: 1 Year of Skilled Home Health Experience Required License: Pending License Accepted Certifications: BLS Required Must-Have: Experience with Oasis Job Description: - Provide nursing care for patients in a home health setting. - Manage a caseload with a focus on high-quality care, visiting up to six patients per day. - Utilize EPIC for charting and ensure accurate documentation. - Coordinate with the healthcare team to develop and implement patient care plans. - Wear any basic color scrubs appropriate for the role. - Ensure reliable transportation is available at all times for travel within a designated area. - Adhere to all health and safety regulations in caring for patients. - Participate in orientation and complete required modules pre-start and annually. - Maintain open communication with supervisors and provide regular updates on patient status. - Be responsive to the facility's system for grouping clients and minimizing travel time. This position does not require weekend work, block scheduling, or on-call duties. Applicants should have two supervisory references verifying one year of employment in a comparable specialty within the last three years. Candidates must be open to floating between facilities as necessary. Float refusal may lead to assignment termination or forfeiture of guaranteed hours. The facility provides communication devices needed to perform duties if not supplied, with reasonable communication expense provisions.
