Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not Guaranteed Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License: License must be in hand Certifications: CPR, BLS Must-Have: - Proficiency in OASIS and HomeCare HomeBase (HCHB) used within the last 6 months or completion of a retraining course - Ability to travel up to 75 miles from the home base - Reliable transportation, driver's license, and agency-required liability insurance Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies - Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Perform home visits and attend periodic office meetings - Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits - Manage an average of 5-8 patients per day - Meet productivity expectations with a minimum of 30 units per week - Responsible for all required documentation to be completed within 24 hours of the patient appointment - Participate in weekend rotation every 4-5 weeks and on-call rotation every 3-4 weeks, depending on staffing needs - Utilize HomeCare HomeBase and Pointcare EMR systems efficiently - Cover defined territory with an expected travel of 200-350 miles weekly Please note: Flexibility with patient locations and ability to adjust based on patient schedules are essential for this role.
