Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00), Monday-Friday Guaranteed Hours: No Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience License: Must have license in hand, no exceptions Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) and Oasis - Reliable transportation, driver's license, auto insurance - Willingness to travel up to 75 miles from the home clinic - Ability to complete documentation within 24 hours - Open to on-call rotations Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Manage a patient load of 5-10 patients per day, equating to roughly 6-8 points - Conduct various types of cases including Start of Care, Wound Care, Follow-up Visits, etc. - Engage in home visits and periodic office meetings - Drive responsibly to patients' homes daily for care delivery - Attend weekly meetings at the clinic - Manage travel and documentation tasks with point values contributing to productivity expectations of 30 points per week - Adapt to weekend and on-call requirements every 4 weeks - Work effectively to hit weekly productivity targets based on assigned patients - Be open to floating to nearby clinics if patient census drops - Maintain patient privacy and follow agency policies and procedures - Receive mileage reimbursement from patient home to patient home
