Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No guaranteed hours; productivity points must be met Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience License: Must have license in hand; pending licenses not accepted Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) with usage within the last 6 months - Proficiency in OASIS - Reliable transportation, valid driver's license, and required auto insurance Job Description: - Provides, coordinates, and directs home nursing care according to physicians' orders and agency policies and procedures through competent application of the nursing process. - Responsible to Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical/Vocational Nurses. - Conducts home visits and attends periodic office meetings. - Assigned between 5-10 patients per day, equating to approximately 6-8 points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Expected to engage in potential weekend rotation and on-call duties as agreed upon in the interview. - Must be willing to travel up to 75 miles from the home clinic; travel between patient homes is compensated at the IRS standard rate, except for the commute to and from the first and last patient. - Responsible for completing all documentation within 24 hours of the patient appointment. - If necessary, may float to a nearby clinic to pick up additional hours if patient census is low. This role involves significant travel and requires adaptability and commitment to ensure the best patient care.
