Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience License: Must have a valid nursing license in hand Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last 6 months - Reliable transportation with a current driver's license and required liability insurance - Flexibility to travel up to 75 miles from the assigned location Job Description: - Provide, coordinate, and direct home nursing care according to physician's orders, based on agency policies and procedures through competent application of the nursing process. - Responsible to the Clinical Manager. - Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Handle multiple types of cases, including Start of Care, Wound Care, and Follow-up Visits. - Manage an average of 5-8 patients per day, with productivity expectations equivalent to approximately 30 points per week. - Complete all documentation within 24 hours of patient appointments. - Engage in weekend on-call rotations as required. - Be open to float to nearby clinics to pick up hours if patient census is low. - Maintain compliance with the dress code by wearing scrubs. Note: Candidates currently employed directly with any related agency as a traveler are not eligible to apply. Must accept assigned patients regardless of distances between appointments.
