Job Title: Hospice Aide/Home Health AideJob Specialty: Rehabilitation Therapy - Hospital Aide, Home Health RNJob Duration: 13 weeksShift: Days, 8:30 am - 5:00 pm (Schedule can be flexible based on patient's availability)Guaranteed Hours: 40 hours per weekExperience: Minimum of 1 yearLicense: Current California driver's licenseCertifications: Current California Home Health Aide Certification, Current CPR certification - American Heart Association BLS Health Care ProviderMust-Have: Automobile insurance with the minimum Bodily Injury Liability Insurance coverage per agency policyJob Description:- Provide personal care, home hygiene services, and other supportive services to patients receiving hospice care at home.- Assist patients with bathing, grooming, and toileting activities.- Monitor vital signs and assist with feeding patients, as specified by the case manager or supervisor.- Maintain a safe and clean environment in the patient's home.- Use assistive devices such as Hoyer lifts, canes, walkers, and wheelchairs.- Offer comfort measures and integrative therapies, including companionship and conversation.- Communicate with the case manager about patient status and participate in care planning.- Assist in training families in caring for terminally ill patients.- Preserve patient dignity and respect spiritual and cultural beliefs.- Perform tasks requested by the case manager to assist patients in obtaining health services.- Utilize electronic devices to access care plans and document visits.- Communicate promptly with patients, families, and department staff regarding scheduled visits.- Attend and participate in interdisciplinary group meetings and training sessions.- Maintain proficiency in providing quality hospice aide care and adhere to agency policies and procedures.- Mentor new clinical providers and assist with their orientation.- Engage in quality improvement activities related to areas of practice.
