Job Description:
- Identify the patient's and family's physical, psychosocial, environmental, safety and developmental needs and individualize the patient's plan of care based on the needs identified.
- Document problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes achieved from intervention and care provided
- Assess all aspects of the patient's pain and develop an individualized pain management plan.
- Participate in the interdisciplinary team (IDT) meeting to establish, review and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
- Maintain up to date patient records to ensure that problems, plans, interventions, and goals are accurately and clearly stated, and changes are reflected as they occur.
- Support the patients and family's unique spiritual and cultural beliefs.
- Provide holistic, family-centered care across treatment settings to improve the quality of life.
