As a Long Term Care Registered Nurse (RN), you will provide specialized nursing care to residents in long-term care settings, focusing on managing chronic conditions, promoting quality of life, and ensuring the overall well-being of elderly or chronically ill patients.\n\nKey Responsibilities:\n\nPatient Assessment and Care Planning:\n\nConduct comprehensive assessments of residents' health status, including physical examinations, medication histories, and psychosocial needs.\nDevelop individualized care plans in collaboration with interdisciplinary teams, addressing residents' medical, social, and emotional needs.\nMedication Administration and Management:\n\nAdminister medications and treatments as prescribed, ensuring adherence to medication schedules and dosage instructions.\nMonitor residents for medication side effects, therapeutic responses, and changes in health status, reporting findings to healthcare providers.\nWound Care and Chronic Disease Management:\n\nProvide wound care, including assessment, cleaning, dressing changes, and monitoring wound healing progress.\nManage chronic conditions such as diabetes, hypertension, and respiratory diseases through ongoing assessment, education, and coordination of care.\nPatient and Family Education:\n\nEducate residents and their families about health conditions, treatment plans, and self-care techniques to promote independence and improve outcomes.\nProvide guidance on nutrition, exercise, medication management, and lifestyle modifications tailored to residents' needs.\nPsychosocial Support and Advocacy:\n\nOffer emotional support and advocacy for residents, addressing their psychosocial needs and facilitating access to supportive services and resources.\nPromote a positive living environment that respects residents' dignity, autonomy, and rights.\nDocumentation and Communication:\n\nMaintain accurate and timely documentation of nursing assessments, interventions, and resident progress in electronic health records (EHRs).\nCommunicate effectively with interdisciplinary team members, including physicians, therapists, social workers, and caregivers, to coordinate care plans and ensure continuity of care.\nQuality Assurance and Regulatory Compliance:\n\nParticipate in quality assurance programs, audits, and regulatory compliance activities to uphold standards of care and ensure adherence to state and federal regulations.\nImplement infection control measures and safety protocols to maintain a safe environment for residents, staff, and visitors.\nCare Coordination and Transitional Care:\n\nCoordinate care transitions for residents moving between acute care hospitals, rehabilitation facilities, and long-term care settings.\nCollaborate with discharge planners and community resources to facilitate seamless transitions and optimize continuity of care.
