J ob Description: Registered Nurse (RN) - Long-Term Acute Care (LTAC) Hospital Position Summary: As a Registered Nurse in our Long-Term Acute Care (LTAC) hospital, you will provide comprehensive nursing care to patients with complex, chronic illnesses requiring extended hospitalization. The ideal candidate will demonstrate exceptional clinical skills, critical thinking, and compassion to help improve the quality of life for our patients and support their families through their healthcare journey. Key Responsibilities: 1. Patient Care: - Assess, plan, implement, and evaluate individualized patient care plans for patients with multiple comorbidities. - Monitor patients' vital signs, fluid intake/output, and overall clinical status, documenting any changes in the patient's condition. - Administer medications and treatments as prescribed, adhering to safety protocols and guidelines. 2. Collaboration: - Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers, to develop and coordinate comprehensive care plans. - Communicate effectively with patients and their families regarding care plans, treatment options, and any changes in condition. 3. Education: - Provide health education to patients and their families on managing their conditions and promoting wellness. - Participate in patient and family support programs as necessary. 4. Documentation: - Ensure accurate and timely documentation of patient care, assessments, and interventions in the medical record. - Prepare and maintain reports and records in compliance with hospital policies and regulatory standards. 5. Quality Improvement: - Participate in quality improvement initiatives and contribute to optimizing patient outcomes and satisfaction. - Stay current with evidence-based practices and implement changes as required to enhance patient care. 6. Compliance: - Adhere to all hospital policies, procedures, and regulatory requirements. - Maintain patient confidentiality and ensure compliance with HIPAA regulations. Qualifications: - Licensure/Certification: - Current Registered Nurse (RN) license in facility state or compact license. - BLS required, ACLS certification preferred - Experience: - 1 year of previous experience in an LTAC, critical care, or similar setting preferred. - Experience with ventilator management, tracheostomy care, and complex wound care is a plus. - Skills: - Strong critical thinking, assessment, and organizational skills. - Excellent interpersonal and communication skills. - Ability to work effectively in a fast-paced and diverse environment. Working Conditions: - 12 hour shifts - Physical demands may include lifting, bending, standing for prolonged periods, and moving patients.
