Job Title: RN - Wound Care /WOCN Certified Job Category: Community Care - NursingDepartment: Home Care AdminLocation: New York, NY - 10002Start Date: 11/25/2024Hours: 40 per week (8 hours per day, 5 days per week)Schedule: 70% in-office; potential travel to patients' homes in Manhattan, Brooklyn, Bronx, and Queens. Wound Care Certification required
Duties:
- Conduct health assessments focused on wound, ostomy, and continence care. Perform physical assessments and develop care plans based on diagnosis and patient needs.
- Recommend appropriate medications and treatments.
- Develop, revise, and evaluate standards of care. Assist in selecting supplies, technology, and equipment.
- Assess and document patient responses to treatment. Evaluate the quality and delivery of care by clinical staff.
- Collaborate with healthcare teams and refer to other disciplines as needed.
- Provide education to patients and families on wound, ostomy, and continence care.
- Develop and evaluate staff education programs, including new employee orientation and continuing education.
- Provide assessments and initiate preventive measures for patients at risk of developing pressure injuries.
- Collaborate with the Assistant Director of Nursing for Wound Care to implement best practices.
- Participate in quality improvement activities at the department and hospital-wide levels.
- Deliver culturally competent education to patients, families, and staff.
- Ensure data confidentiality in accordance with patient rights policies.
- Maintain professional competency through ongoing clinical activities.
- Identify and refer patients to community resources.
- Manage assets related to rented equipment, such as specialty beds and negative pressure devices.
Required Skills & Experience:
- Minimum 2 years of experience in medical/surgical nursing.
- Experience with wound and colostomy care.
- Excellent communication skills.
- Knowledge in community outreach and wound management.
