AlliedTravelCareers is working with Cynet Health to find a qualified Physical Therapist (PT) in Agawam, Massachusetts, 01001!Pay Information$2,374 per weekAbout The PositionJob Title: Physical Therapist - Home HealthJob Specialty: Physical Therapy - Home HealthJob Duration: 13 weeksShift: Day, 5x8-Hour (08:00 - 17:00)Guaranteed Hours: No guaranteed hoursExperience: Minimum 1 full year of Adult Home Health experienceLicense: Must have license in handCertifications: CPR, BLSMust-Have: Proficiency in HomeCare HomeBase (HCHB) EMR system and Oasis for RNs, PTs, and OTsJob Description:- Provide, coordinate, and direct home nursing care based on physicians' orders and agency policies through the competent application of the nursing process- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses- Conduct home visits and periodic office meetings; must have reliable transportation, current driver's license, and agency-required liability insurance- Average number of patients per day: 5-8 with varying types of visits including Start of Care, Wound Care, and Follow-Up Visits- Travel up to 75 miles from the home clinic, with mileage reimbursement at the IRS Standard rate- Proficiency in Oasis and HomeCare HomeBase (HCHB) required; retraining course necessary if not used within the last 6 months- Accept all patients on the schedule, complete documentation within 24 hours, and meet weekly productivity points of 30- Must be open to on-call rotations and weekend requirements as per location and profession- Position reports to designated clinic in western Massachusetts, covering territories including The Hamptons, Williamsburg, Hatfield, Hadly, Amherst, Pelham, and surrounding areas- 5x8-Hour schedule with possible weekend rotations and on-call responsibilities, depending on location; every 5th weekend rotation typically- Orientation required, but no guaranteed hours; contractor may be floated to nearby clinic for additional hours if needed- Local and first-time travelers accepted, with contractors expected to drive to patients' homes daily and clinic for meetings- Contractor responsible for meeting weekly productivity points, with no guaranteed hours; opportunity to float to nearby clinic if patient census is low.9605502EXPPLATJob RequirementsRequired for Onboarding
