Job Title: Home Health Physical Therapist
Job Location: Florida
Type of Facility: Home Health Agency
Length of Assignment: 13 Weeks (open to 26 weeks)
Start Date: Early January
Work Schedule: 8-10 hour shifts for 4-5 days per week
Roles and Responsibilities:
- Provide in-home physical therapy services to homebound geriatric patients
- Conduct assessments, evaluations, and treatments
- Develop and implement initial and ongoing treatment plans
- Create discharge plans and supervise assistants/aides
- Manage a caseload of 30-32 patients per week
- Schedule and coordinate patient visits independently
- Communicate treatment plans with patients' families and other healthcare professionals
- Adapt to different work environments and be tech-savvy
- Handle occasional cancellations and follow-up as needed
Requirements:
- Florida PT license
- CPR certification
- Valid driver's license, car registration, and insurance
- At least 1 year of home health experience
- Own smartphone, tablet, or laptop for documentation
- Adaptability and strong communication skills
- Lymphedema experience and bilingual skills are a plus
