Physical Therapist This position will function as part of our start-up and survival team (S&S). As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Team members will manage the initial setup and transition to the permanent clinical team through onboarding and training. They will be or will become MatrixCare power users. Our team ensures seamless operations during growth, extended PTO, or other gaps. These team members prevent extra strain on the team, keeping patient care our top priority. This position will require a person to travel to different states and sites on a weekly basis. Responsibilities:
- Provide exceptional physical therapy services to patients by assessing and understanding the care plan.
- Evaluate patient's functional status and home environment for hazards or barriers.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Maintain and process all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
- Submits all clinical documentation, time records and other information in the required time frame.
Qualifications:
- Current and active PT license in the state of employment.
- We are seeking individuals who either hold a PT license or are willing to obtain one in the following states: CT, NY, MA, NH, ME, TX, OH, NJ, PA, and VA.
- Graduate degree or greater in physical therapy.
- Minimum of 1 year of experience in certified home health.
- In-depth understanding of end-of-life care principles and practices and certified home health.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
