About The Position**Job Title:** Physical Therapist**Job Specialty:** Occupational Therapist**Job Duration:** 13 Weeks**Shift:** Monday - Friday, 8:00 am - 4:30 pm; weekends as needed**Guaranteed Hours:** Not specified**Experience:** Minimum of one year of experience in a long-term care/home care setting as a Physical Therapist; recent home care experience within the last year**License:** Possession of a current New York State License in Physical Therapy**Certifications:** Not specified**Must-Have:** Home health experience within the last year, one year of PT experience in a home health setting**Job Description:**- Provide case management and direct patient care within a team environment.- Administer comprehensive physical therapy modalities in the home setting.- Assist in evaluating the level of function and help develop and revise the care plan as necessary.- Prepare clinical and progress notes and consult with the family and other agency personnel.- Promote organization philosophy, mission, and policies to ensure the delivery of quality care.- Develop and revise treatment and discharge plans in conjunction with the patient, family, physician, and care delivery team.- Initiate appropriate rehabilitative programs including gait training, transfers, ADLs, home modification, care, and use of equipment.- Evaluate patient's progress and make necessary adjustments to the physical therapy program.- Oversee the quality of patient care services and ensure student health, safety, and compliance with the plan of care.- Educate patients, families, and team members on disease processes, self-care techniques, and preventive strategies.- Supervise physical therapy assistants to ensure delivery of quality care.- Display respect for patients' privacy, confidentiality, and property.- Interact positively with patients, physicians, referral sources, and other stakeholders.- Participate in patient conferences and assist in the orientation of new rehabilitation personnel.- Maintain comprehensive clinical records and refer to appropriate community agencies/resources.- Demonstrate flexibility, enthusiasm, and willingness to cooperate with others.- Contribute to an environment of professional growth, learning, trust, and mutual respect.- Participate in education, professional organizations, and continuing education opportunities.- Monitor and strive for continuous improvement across all aspects of the physical therapy function.- Ensure practices meet agency performance competencies, quality management, and regulatory guidelines.9469903EXPPLAT
