**Job Title:** Rehabilitation Therapy - Physical Therapy Assistant**Job Specialty:** Physical Therapy Assistant**Job Duration:** 13 weeks**Shift:** Monday-Friday, Days**Guaranteed Hours:** 32 hours per week**Experience:** Accepting new graduates**License:** State License Verification Required**Certifications:** CPR**Must-Have:** - Resume with License Verification- Three References**Job Description:**- Split responsibilities between two facilities.- Rehab Optima experience is preferred.- Conduct county criminal background check for the past 7 years for all counties of residence and employment.- Conduct federal criminal background check for the past 7 years or more.- Conduct multi-state criminal search for the past 7 years or more.- Conduct nationwide sex offender search for the past 7 years or more.- Conduct HHS/OIG search.- Conduct GSA search.- Perform Social Security Number Trace.- Complete 1 Step PPD or Quantiferon test.- Conduct SAM search.- Complete background check in the relevant state.- Ensure Hepatitis B vaccination.Please ensure the prescreen is filled out and attached upon submission of the candidate.
