**Job Title:** Physical Therapy Assistant**Job Specialty:** Rehabilitation Therapy - Physical Therapy**Job Duration:** 13 weeks**Shift:** Monday - Friday, Days**Guaranteed Hours:** 32 hours per week**Experience:** New graduates are welcome; however, experience in Skilled Nursing Facilities (SNF) is preferred.**License:** State License Verification required**Certifications:** 1 Step PPD or Quantiferon or Proof of positive PPD and Chest X-Ray**Must-Have:** - Covid test required 72 hours prior to start date with results issued 48 hours prior.**Job Description:** - Ensure completion and attachment of the prescreen form upon candidate submission. - Attach candidate's resume and license verification. - Conduct and attach the following checks: - Three References - County Criminal (7 years for all counties lived and worked) - Federal Criminal (7 years or more) - Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS/OIG Search - GSA Search - Social Security Number Trace - SAM Search - Engage in patient rehabilitation therapy sessions under the supervision of a Physical Therapist. - Assist with the execution of personalized therapy plans. - Provide support in tracking patient progress and updating records accordingly. - Facilitate communication between patients, families, and healthcare providers. **Additional Information:** - Position urgency is normal. - Bilingual capability is not required. - Holiday coverage is not required.
