TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Webster, New York, 14580 Pay Information $2,764 per week About The Position Job Title: Level 2 CHHA Case Manager RN Job Specialty: Case Manager Registered Nurse Job Duration: 13 weeks Shift: Monday to Friday, 8:00am - 4:30 pm, Day shift, weekends as needed Guaranteed Hours: Not specified Experience: One year of experience as a clinical nurse in a hospital required; 9 months experience in home care preferred. Certified home health care experience required. OASIS experience required. License: RN License (New York License ONLY) Certifications: BLS (any provider accepted) Must-Have: - Certified home health care experience - EPIC or Netsmart experience preferred - Strong critical thinking skills - Ability to follow standards/procedures and make appropriate clinical judgments - Strong interviewing, clinical assessment, and communication skills - Ability to exercise initiative and independent judgment - Ability to build and maintain professional relationships - Current NYS driver's license - Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments Job Description: - Develop and revise treatment plans and discharge plans in collaboration with patients, families, physicians, care teams, and third-party payers. - Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in the condition within defined time frames. - Process orders and notify physicians of patient needs and condition changes; complete certification/recertification orders and discharge summaries. - Ensure a respectful environment that promotes patient privacy, confidentiality, and property. - Initiate appropriate preventive and rehabilitative strategies. - Provide services requiring specialized nursing skills. - Consult with and educate patients, families, and other team members about disease processes, self-care techniques, and preventive strategies. - Provide supervision of LPNs to ensure patient health, safety, and compliance with care plans. - Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the agency. - Interact with patients, physicians, and referral sources to maintain positive relationships. - Support care management philosophy through collaborative planning and case management. - Ensure completion of all clinical records for compliance with legislation and seamless service delivery. - Maintain knowledge of available community-based services. - Demonstrate flexibility and willingness to visit patients based on need rather than geography. - Resolve issues at the point of contact. - Obtain necessary authorizations and ensure the release of information forms are signed. - Attend patient care conferences for assigned patients. - Participate in achieving agency operational and financial objectives, including monitoring quality indicators and customer satisfaction. - Understand the impact of nursing functions on patient outcomes and agency productivity. - Participate in quality management reviews and implement corrective actions. - Contribute to professional growth and a respectful work environment. - Engage in educational programs and demonstrate professional growth and competency. - Be responsible for achieving Key Performance Indicators (KPIs). - Perform other duties as requested. 26517340EXPTEMP Job Requirements Required for Onboarding Pharmacology Skills Checklist Specialty Exam 5 Benefits of Travel Nursing Adapting to new facilities and teams challenges your problem-solving skills.
