The Resource Center has been providing services to persons with disabilities in the Chautauqua County area since 1958. From our humble beginnings as a gathering of parents and concerned citizens who wanted to provide educational and training opportunities for persons with developmental and intellectual disabilities, The Resource Center has grown into a comprehensive agency providing services to thousands of persons with all types and levels of disabilities.The Resource Center is the largest non-governmental employer in Chautauqua County with more than 1,300 employees on its payroll.The mission of The Resource Center is to support individuals with disabilities and other challenges in achieving maximum independence, contributing to their community, experiencing lifelong growth, and enjoying quality of life.DIETICIANRESPONSIBLE TO:Residence Coordinator Supervisor/designeeFUNCTION:To plan, develop, coordinate and document the delivery of dietetic services to an assigned caseload of residents, based on the developmental needs and strengths.REQUIRED KNOWLEDGE SKILLS AND ABILITIES:Must possess and demonstrate a firm understanding of the principles of Person-Centered Planning, Ability and willingness to advocate on behalf of persons being supported, as needed; Willingness, ability and commitment to increase professional knowledge base and remain at the forefront of current philosophies within the field through attendance at trainings/conferences in order to successfully and creatively support people in attaining their life goals. Must possess a working knowledge of applicable Federal and State regulations; Must be able to self-direct, self-initiate, problem-solve, and effectively facilitate; Must be flexible and willing to work non-traditional hours, as needed or assigned to meet consumer/customer need; Excellent verbal and written communication skills across various channels is essential; Strong organizational skills are critical; Strong computer skills and knowledge of or ability to learn and utilize software packages including Outlook, Word, Excel, and Therap; Physical condition or reasonable accommodation(s) commensurate with the demands of the position.MINIMUM QUALIFICATIONS:Bachelors degree in food and nutrition from an accredited program, registered or eligible for registration by the American Dietetics Association. One (1) year related experience preferred.SPECIFIC DUTIES:1.Presents the dietary plan, and/or interventions needed at all annual Individual Program Plan meetings for assigned residents and quarterly meetings as deemed necessary.2.Coordinates dietetic services with the plans and interventions of other disciplines within the Intermediate Care Facility, as an integral part of the interdisciplinary team.3.Confers with those staff responsible for medical care and treatment for the purpose of gaining clinical information that could impact on nutritional status including, but not limited to, food/drug interaction, chewing/swallowing problems, and chronic diseases.4.Ensures implementation of all modified and special diets, in conjunction with the residents' interdisciplinary team, including a prescribing physician.5.Ensures that the Mealtracker program is kept current for each individual, in conjunction with the individuals' interdisciplinary team, including a prescribing physician.6.Charts on individual case records regarding weights and other pertinent data at least monthly. Completes individual assessments on all new admissions and any reassessments thereafter.7.Provides periodic on-site observation and monitoring to ensure that dietary plans are carried out in a manner consistent with the developmental level of each resident. Also, to ensure maintenance of health and sanitary standards in compliance with state and local regulations.8.Ensures that menus provide a variety of foods at each meal, reflect variety for the season of the year, and are prepared with the involvement of the individual to the maximum extent possible.9.Provides orientation and ongoing training to food service personnel and training to direct care staff to assist in the provision of Dietetic Services.
